TERMINATION OF PREVIOUS MM2H SOCIAL VISIT PASS OF MAIN APPLICANT
1. Letter of intent to terminate from MM2H program.
2. Principal’s death / divorce certificate.
3. Copy of principal’s passport (particular page and MM2H social visit pass page).
4. Copy of dependent’s passport (particular page and MM2H social visit pass page).
5. Copy of Conditional Approval Letter.
6. Copy of Fixed Deposit Certificate

APPEAL TO CHANGE PRINCIPAL
APPROVED UNDER FIXED DEPOSIT CRITERIA
1. Letter of intent to change principal.
2. Termination letter from Ministry of Tourism and Culture.
3. Copy of previous conditional approval letter.
4. Copy of principal’s passport (particular page and MM2H social visit pass page).
5. Copy of dependent’s passport (particular page and MM2H social visit pass page).
6. Letter of Good Conduct from Police Department (country of origin).
7. MM2H application form (1 copy per person).
8. IM.12 (3 copies).
9. IM.38 (if any – 1 copy). Compulsory for visa required country such as Bangladesh, India, Pakistan, etc. Refer www.imi.gov.my for further details.
10. Authorization Letter (for bank verification purpose).
11. Principal’s death / divorce certificate.
12. Fixed deposit certificate under the MM2H program (if any) or savings account showing the ability to open a fixed deposit account (if applicable)
13. Original letter from the bank to confirm the said Fixed Deposit account.
14. RBII form (medical report – complete with doctor’s signature and hospital’s/clinic’s stamp).
15. Security Bond / Personal Bond Form (complete with stamping RM10 at Inland Revenue Board of Malaysia).
16. Medical insurance (optional for age 60 years and above, compulsory for age below 60 years old).
17. Photos (4 copies).
APPROVED UNDER MONTHLY GOVERNMENT PENSION
1. Letter of intent to change principal.
